I give permission to Cave Spring United Methodist Church for the follow:

Use of my child’s image, by means of digital or film photography with respect to all Cave Spring United Methodist Next Generation Ministries activities and events. 

• Use of my child’s image in any printed publications from the church.
• Use of my child’s image in any electronic publication from the church.
• Use of my child’s image on any social media page created for or by the church.
• Use of my child’s image on any website created for or by the church.

By signing this agreement on behalf of a minor child, I warrant that I am the legal parent or guardian of the child and have full legal authority to sign this agreement on behalf of the child.
